All those traveling outside the United States now need a passport to re-enter the country, including those traveling to Canada, Bermuda and the Caribbean. For more information, visit the U.S. Department of State.

SPECIAL NOTICE
Beginning October 30, 2010, the United States Department of State will not accept Puerto Rican birth certificates issued prior to July 1, 2010, as primary proof of citizenship. To read more about this change visit the U.S. Department of State website.
New Applicants
You must apply in person and complete the Passport Application Form DS-11 if any of the following
is true:
You have never had a U.S. passport
You are under age 16
Your most recent passport was issued more than 15 years ago
Your most recent passport was lost or stolen
Please bring the following:
Previous U.S. passport
Certified copy of your birth certificate**
Certificate of naturalization
Previous U.S. passport
Valid driver's license
State identification card
Certificate of naturalization
Recent color photo
Front view

The Clerk's Office can provide a photograph for $10.00
**Please Note: Beginning April 1, 2011 all birth certificates must include the full names of the
applicant's parent(s).
Passport Renewal
Renewal is done by mail. You must complete Form DS-82. You may apply by mail if:

Include your most recent passport with a new passport photo
If your name has changed, submit documentation of that change, i.e. a court order or marriage certificate
Passport Reapplication for Name Change
If your passport was issued less than 1 year ago, use Reapplication Form
DS-5504
If your passport was issued more than 1 year ago, use
Form DS-82
Passport Card
You may apply for a passport card, which may only be used for land and sea travel between the United States and Canada, Mexico, Bermuda and the Caribbean.

Passport service is also available Tuesday-Thursday only, 8:00 am - 4:30 pm at our Goldenrod location.
Fees
Fees are paid to both the Department of State and the Clerk's Office. Please note the various forms of payments accepted.
Adults are defined as 16 years of age and older.
Children are defined as 15 years of age and younger.
