Passports

All those traveling outside the United States now need a passport to re-enter the country, including those traveling to Canada, Bermuda and the Caribbean. For more information, visit the U.S. Department of State.

Anticipated Time to Receive Passport

Expedited: 2-3 weeks

Regular processing: 4-6 weeks

SPECIAL NOTICE

Beginning October 30, 2010, the United States Department of State will not accept Puerto Rican birth certificates issued prior to July 1, 2010, as primary proof of citizenship. To read more about this change visit the U.S. Department of State website.

New Applicants

You must apply in person and complete the Passport Application Form DS-11 if any of the following
is true:

  • You have never had a U.S. passport
  • You are under age 16
  • Your most recent passport was issued more than 15 years ago
  • Your most recent passport was lost or stolen

Please bring the following:

  • Proof of citizenship (one of the following) 

          Previous U.S. passport

          Certified copy of your birth certificate**

          Certificate of naturalization

  • Proof of identity (one of the following)

           Previous U.S. passport

           Valid driver's license 

           State identification card

           Certificate of naturalization 

  • A passport photo

           Recent color photo

           Front view

           2 inches X 2 inches

           The Clerk's Office can provide a photograph for $10.00

**Please Note: Beginning April 1, 2011 all birth certificates must include the full names of the
   applicant's parent(s).            


Passport Renewal 

Renewal is done by mail. You must complete Form DS-82. You may apply by mail if:

  • You hold a passport and it is not more than 5 years past the expiration date
  • You were at least 16 years old when it was issued

How to renew by mail:

  • Complete and sign Form DS- 82
  • Include your most recent passport with a new passport photo
  • If your name has changed, submit documentation of that change, i.e. a court order or marriage certificate

Passport Reapplication for Name Change  

  • If your passport was issued less than 1 year ago, use Reapplication Form DS-5504
  • If your passport was issued more than 1 year ago, use Form DS-82

Passport Card

You may apply for a passport card, which may only be used for land and sea travel between the United States and Canada, Mexico, Bermuda and the Caribbean.

Minor Applications

Complete Form DS-11

  • Proof of citizenship and identity are same as for adults
  • The minor must appear in person
  • Both parents must appear together to sign the application form. If only one parent appearing, the absent parent must submit a notarized Statement of Consent (Form DS-3053) and a photocopy of the front and back of their driver's license. The consent form cannot be more than three
    months old

  • Winter Park branch: 450 North Lakemont Avenue
  • Apopka branch: 1111 North Rock Springs Road
  • Ocoee branch: 475 West Story Road, Ocoee

Passport service is also available Tuesday-Thursday only, 8:00 am - 4:30 pm at our Goldenrod location.

Fees

Fees are paid to both the Department of State and the Clerk's Office. Please note the various forms of payments accepted.

Adults are defined as 16 years of age and older.

Children are defined as 15 years of age and younger.
